How do I get prescriptions refilled?
Prescription refills are handled during regular office hours Monday through Friday, 8:00 a.m. to 5:00 p.m. only. In order for your physician to review your medical history, please allow up to 48 hours to process your refill request. Please understand that any calls received on Friday will not be processed until the following week. For prescription refills, please call (510) 763-0884.
My physician has requested special tests for diagnostic purposes. Do tests such as a Nerve Conduction Study, EMG or MRI need prior authorizaton?
All of these special tests will require prior authorization from your insurance carrier before our office can make arrangements with the appropriate facility regarding scheduling an appointment. The authorization process may take 2-6 weeks. It would be beneficial if you, the patient, also contacted your insurance company regarding the benefits of these special tests for diagnosing your hand or upper extremity problem.
What should I do in an emergency regarding my medical care?
Our physicians are on call 24 hours a day, seven days a week to care for all of your orthopaedic needs. For non-life-threatening emergencies, please call (510) 763-0884.
How do I get copies of my medical records?
Our office must have a release form signed by the patient before any records can be released to a patient or outside provider. Release forms can be obtained by calling, visiting our front desk or downloading the medical release form. Patients may fax the medical release form or letter requesting medical records to 510-763-8753.
The fax should include: name, date of birth, signature, and complete mailing address of where records should be sent. Please be aware that it may take up to three business days to process your request.
Our standard release of information includes the following:
- Physician’s dictated notes
- Therapist’s dictated notes
- Copy of digital X-rays (a service fee will be charged)
